Do you think niche SEO (like healthcare or local services) performs better when handled by specialists rather than general agencies?
 
SEO is a in need of constant daily, weekly, monthly, and annually efforts. It' a battle. You need to learn to do SEO for yourself until such time as you can afford a freelancer to manage it for you. Once the demands for SEO exceed the capacity of a freelancer you ten should look at other options.

If you are looking for SEO to be your primary traffic source, you will not succeed. SE traffic is never, NOT EVER, a successful primary traffic source.

I think specialists definitely have an edge because they understand the specific compliance and keyword intent of that niche. However, a general agency with a strong focus on manual link building can also get great results if they do deep research.
