The penalty can be a byproduct of an algorithm update or an intentional penalty at the hands of the negative SEO / Black Hat SEO.

If we are not following the Google algorithm, it gives penalty which means it pull down your website rank on the search engine result page.

Google defines some algorithms and rules with the passage of time they will update the previous algorithms and also launch some new updates if any website does not fulfill all the requirement of that algorithm then google penalize that website and decrease ranking.
 
What to do to protect from negative seo ?
There are a number of things you can do to protect against negative SEO:

1) Configure Google Webmaster Tools Email Alerts. This will mean you'll get alerted when your site is attacked by malware, having server problems, big traffic changes etc
2) Ensure that your site logins are secure, consider setting up two-step authentication
3) Be sure to keep an eye on your backlinks, Ahrefs is a great tool to help you do this

The Fix
  1. Navigate to Google Search Console > Crawl > Fetch as Google, then fetch pages from the affected portions of your website.
  2. Compare the content on your web page to the content fetched by Google.
  3. Resolve any variations between the two so they end up being the same.
  4. Check all redirects and remove redirects that:
    • Send users to an unexpected destination.
    • Conditionally redirect (ex: only redirecting users coming from a certain source).
    • Are otherwise “sneaky”.
  5. Submit a reconsideration request after fixing these issues.
Pro Tip: These types of redirects are often created by CMS plugins, may be in your .htaccess file, or could be written in JavaScript.
