Changing old post URLs and titles- Any WP plugin or...?

Hi guys

When last year I started my blog, I knew nothing about seo and how it all worked, so when making titles for my blog posts, I really messed up. Now that I have learned a little more, I am trying to go back and fix them and optimize them a bit more. However, I am afraid of changing titles and urls.

Is there any plugin or a methods that would allow me to change my url and or tiles without losing the traffic or causing a "not found" situation?

Also, if I change urls to a more optimized version, does it help with seo and perhaps ranking better or do search engines will still rely on the original url?

I used this one a couple of years or so back: WordPress › Dean's Permalinks Migration WordPress Plugins

It hasn't been updated so it's unknown whether it will work with current versions of WordPress but worth a try. It did work well when I needed it.
