How to monetize an e book with email submit form?

I wrote an ebook. The topic is really popular and many books are written in English. But there is no too many books written in my language (Serbian/Croatian/Bosnian). Now I am looking for a CPA network to monetize my book.

Many people in my country are able to read in English, but most of them don't speak English. Most download will come from former Yougoslavia countries. I need a CPA network that pays for all countries.

There is another reason I want to use content locker. On-line shopping is not popular in my country. Many people don't have credit cards. Also, few bucks might be too much for them to pay. I won't be able to sell to many copies and make money that way. I want to give it for free and make money.

I need a CPA network where I can ask users to provide email in order to receive a free copy. Aweber has that kind of form, but I don,t want to create email list.

You won`t find any email submits for Serbia/Croatia/Bosnia.
You can only find few higher paying pin submits in CPAGrip for those countries.
