Does flat rate buyer always get first impression?

I found this when I browse big tube site like pornhub, youporn. The first impression ad always comes from the same dude, and he seems out of bidding system since I still got second impression even if I outbid the second position a lot. So I assume he's buying flat. Does flat rate buyer always get first impression? Or there is another channel don't go public?

Most probably its about flat rate/direct deal, if you know that both you and the other guy have same targeting. Sometimes publishers agree to sell first unique impression on a flat rate/pre-paid basis and the rest goes to networks but it totally depends on the agreement between network and publisher
