Affiliate manager contract/agreement

Hi everyone... I need a bit of help...

I've been given the task of running a merchants Affiliate marketing campaign, assuming the role of Affiliate manager I suppose. Is it customary to draw up a contract or agreement for such a thing? If so, would anyone either care to share or possibly recommend a source (internet attorney?) for a sample contract or agreement?

We've had people ask about affiliate management contracts before but I've never seen anyone get an answer on how or where to get one.
