Without knowing anything about your niche, I'd suggest you setup a retargeting list then test Facebook call ads. You have to do them at a "local level" but I've had quite a bit of success with them.
Other then that, you should test out local newspapers, yellow pages, blogs, directories etc...
You can do both.
Personally, I would recommend that you have your own tracking number, that way it gives you the power to swap the offer phone number if you need to. If you send traffic directly to the number that your affiliate networks provides you, you lose some of the control.

If you are running pay per call on PPV (tool bar traffic) for say a plumber...
What I would do is use scrapebox, scrape EVERY plumber website I could local + national. Then bid on those domains on PPV for my plumber pay per call...
I've been running social media retargeting for my pay per call campaigns.

Yes I highly recommend running your pay per call campaigns through your own number.
I've even caught the lead buyers doing shady stuff... "Sorry I have bad reception let me call you back"

Like others have stated PPV is going to be a tough one with pay per call. You have to think of the intent of people visits sites that have PPV adcode. You can, however, ask the visitor questions and drop a pixel based on their answers and retarget them later ;).
I've noticed very few people actually have their phone number on Facebook. So you have to collect a very large list to make it work.
Far as the ringless voicemail that is something I'll definitely have to look into. Sounds very interesting! Have you done any RVM drops?
If it's local and I know i'm sending the calls to a business owner who may not always be able to answer the phone IE. "1 man companies" in HVAC, Plumber, Roofing, etc then I'll hire someone off crags list to answer the phone, collect data from the caller before trying to connect with the correct...
Personally, I use Callrail, simply because I've been using it for so long now. It is a bit pricey as it's $30 month for 10 local phone numbers + $3 per extra local line after that. So it can get a bit pricey if you are using a Ringpool for dynamic tracking.
OpenVBX is a good option as well...
It really depends on your niche and which offer you are running. My best advice is to think really hard about where the audience that would most be interested in your offer hangs out... That is where you need to be advertising ;)
Well I wouldn't say there is a "best" but each source has their own strengths.
I do highly suggest looking up the local new paper and business journal to see if they sell online ads. I can usually get a spot for $10-25 for an entire month depending on the size of the city.
