The vast majority of networks that do ppcall do not accept offshore call centers due to the high % of fraud. One a case-by-case if our advertiser/partner will accept that traffic we can set something up but its pretty rare.
when using the term 'click to call' some people might assume that's referencing google's click-to-call ads when it could instead be talking about people using their phones and clicking on a link on the website that loads the dialer for them....

I'd agree with TJ. Free organic traffic takes so long to develop for pay per call most people don't bother.
Start off with google call-only ads. I'd say once that is established as profitable you should look to Bing for scaling - less traffic but better ROI (you do need a good lander there though).
if you have a successful adwords campaign you should be able to copy it right over to Bing and do well. You'll have less calls but higher ROI typically.
Yep google is usually considered the best... but not the only place you can make a nice profilt. Bing is a not bad place to get your feet wet learning as their click costs are lower.
