I would say that functionality needs to be built into the form on each site using a hidden input field that describes the site that gathered the information. That would require some javascript code. Hope that makes some sense.
Yes, I agree with MarkAse and nicolasthun. As a case in point, I have a website that has ranked number one for my keyword for the past couple of years with a PR of n/a while other sites below it have a PR of 0.
