How to setup prosper202 with "Simple landing page"

I tried to setup tracking use prosper202 with simple landing page but it doesn't work for me. I am so confusing and discourage at the moment .
The problem is prosper202 doesn't tracking the keyword and subID.When I checked in visitors page I haven't seen any keywords and subIDs. But I received clicks in overview page.
I don't know what I did wrong. I am going explains how I had setup in prosper202.

Step #3campaign
I just added my affiliate URL link http://trkur.com/trk?o=6903&p=35419&s1=[[subid]]

Step $4Landing Pages I choose Simple landing
Added my LP nick name as offer name.
LP URL I added my landing page URL.
This is what I added for LP URL "http//pagesurveys.com/index.php"

Step #6Get landing page code
I copy 2 script from them first is Java-Script I put in above in my land page
another one is PHP script. I create a new file for contains the code it's call go.php.
Then I make to PHP file link like this a href="go.php".

Step7 Get Link for promoting
Method promote I choose landing page.

this is my link which generate from step7
"http://xxx.pagesurveys.com/index.php?t202id=9406&t202kw="

I tried to check it's working or not by press some keyword behind the URL link
ike this "http://xxx.pagesurveys.com/index.php?t202id=9406&t202kw=Test"

I return to checked the result in my prosper202 , but there are no keyword and subID. in the visitors.

I am really need help please if you can help me I would be greatly appreciate!!.

try putting the javascript code right above your </body>, it shouldn't be above LP
