I wouldn't use fast and adsense in the same sentence unless you have a very popular site with tons of organic traffic and you plug it in, but if you are building from scratch it will take time 99.99% of the time.
Some offers need approval of your creatives, that is the deal the network made with the client. But there are other offers where you do not need such approval. You have to read the description for the offer so you are complaint. If in doubt contact your AM and he/she will help you out.
I'm not 100% sure what it is but my guess is there is no default page to show so put a index.html that should stop everything from showing. (I'm not a big network guy but that usually does it for me)
