I use tableless, unless I am displaying tabular data. I agree that just using DIVs doesn't make content semantic. Semantic content comes from making use of TITLE, H1 ... H6, lists, strong, emphasis and link content then thinking about the look and feel afterwards.
My view is that page titles, like URLs, should be permanent. If the content of the document is dramatically different, then maybe it should be a new pages with a new title and a new url.

No - Google UK will return results from .com domains. Go to google.co.uk and search for "site:[yourdomain].com" and you'll see which pages in your site are indexed by Google UK.
